How to Become a ZACH Member

Hospitals and clinics wishing to join the Zimbabwe Association of Church-Related Hospitals (ZACH) are required to submit a formal application together with supporting documentation outlined below.

Required Application Documents

The following documents must accompany your application letter:

  • Organisation / Church Profile
  • Organogram (current structure)
  • Board Membership List
  • PVO Registration Certificate
  • Health Professions Authority (HPA) Registration Certificate
  • Ministry of Health Registration Certificate
  • Practising Certificate of the Medical Doctor / Sister-in-Charge (SIC)
Submission Instructions

All application documents should be submitted via email to info@zach.org.zw with the subject line: "ZACH Membership Application".
